Vision Services: CHIP Benefits
Eye Examination
Chip members are entitled to one (1) routine eye examination each benefit year (based on the month in which you first become a CHIP member) at an NVA Participating Vision Care Provider, (co-payments may apply).
Frames
If vision correction is required, you may receive a frame from a selection of approved Cook’s Children’s Health Plan frames at each provider office. If you elect to receive your eyeglasses at a Wal-mart location you may select any frame from the approved collection based on Everyday Low Pricing.
Lenses
CHIP members may also receive standard lenses in any prescription, including polycarbonate material at no cost. Certain lens options are also available when Medically Necessary and approved in advance by NVA.
Other lens and frame options are not covered and if selected are your financial responsibility. The contracted laboratory will not accept orders that include non-covered items.
Replacements
Replacement for lost or broken eyeglasses and when certain prescription requirements are met, is available.
Contact Lenses
Medically Necessary Contact Lenses for certain qualifying Medical Condition are included and will be covered in full.
Cosmetic Contact Lenses (for vision correction only) are also covered, but are in lieu of eyeglasses. Members receive an allowance of $75 towards their purchase along with the fitting and follow-up care, but must pay the providers charges over the $75 allowance. (Certain age and other restrictions apply)
Medical Treatment
Your NVA Participating Vision Care Provider may also provide services when you have a medical eye condition that is treatable by an optometrist.
