With NVA, members have access to a national network of nearly 100,000 vision care access points, in private practice and at national, regional and local retail chains across the U.S. including Puerto Rico.

What types of vision plans are available?

NVA has the flexibility to deliver almost any type of benefit design or funding arrangement. We can match current plan designs or recommend customized options based on actuarial analyses.

Do members need to file claims for benefits?

If an NVA member uses an in-network vision care provider there is no need for claim forms. However, if the member’s plan has out-of-network benefits and the member goes to an out-of-network eye care professional, they will need to complete and send an NVA Direct Reimbursement Claim Form along with a copy of their receipts to NVA to be reimbursed for services and materials.

What is the minimum group size you will quote?

We require a minimum of 10 enrollees, depending on funding arrangement except for FL, where we require at least 51 eligible employees.
