- How can I find a provider in the NVA network?

To get the provider list for your plan, log in to this website or the NVA app and click on the Find a Provider link. You will need to insert a zip code and up to a 30-mile radius for your provider search. If you don’t have time to log in you can click on the find a provider icon and do a search. Just make sure your group/sponsor ID number found on your NVA ID card is readily available.
- What if my vision care provider doesn't participate in my plan?

NVA’s national network of providers includes nearly 100,000 access points of care in all 50 states and Puerto Rico. It’s comprised of independent practitioners as well as optical retailers. By visiting a participating provider location you’ll always get more for your money.
You can easily check to see if your provider participates by calling us toll-free or using the provider locator on this website. If your provider is not currently one of our providers, contact us and we will invite that provider to join the NVA provider network.
If you do decide to use a provider that is not part of the NVA network, your plan may offer reimbursement of a percentage of the money you spent on covered services. Simply print the Out-of-Network Claim Reimbursement Form, complete it, attach your receipts, and mail it to NVA for processing.

- How do I submit a claim?

If you visit an in-network provider you do not need to submit a claim. If you visit an out-of-network provider and if your plan has an out-of-network benefit, you must pay for your services at your appointment. After your service, log in to this website and select “submit a claim” to be reimbursed. If your plan permits, complete and submit the online form. If the online form is not available, print out and complete a hard copy form found outside of log in. You must include an itemized, paid receipt(s) and send back a hard copy form via e-mail, mail, or fax to NVA for processing.
- How do I use my NVA Vision Benefit?

Using your NVA vision benefit is easy. Here’s all you need to do:
1. Select a Provider
Log in and use our NVA Smart Buyer® provider search tool to help you save money OR if you’re in a rush simply click Find A Provider. You can call us anytime and we will find a provider for you.
2. Schedule an Appointment
Contact the participating provider of your choice, let them know your vision program is with NVA, and schedule a convenient time for your visit.
3. …and go.
The participating provider will do the rest.
You can order contact lenses online at http://www.contactfill.com and apply your NVA vision benefit towards the order. The NVA website provides you with a host of online features including provider selection, eligibility checking, and plan coverage.
- Does NVA offer any additional discounts?

Yes we do. You have the option of buying discounted contact lenses from Contact Fill, where you can apply your NVA vision benefit to your order of contact lenses.

Your vision benefit also comes with discounts on hearing health care services and hearing aids.
If you have used up your benefit within your plan period you can use NVA’s EyeEssential® Plan. Receive additional discounts on in-network purchases during the plan period. Some exclusions do apply and the EyeEssential® Plan is not available in every state.

- Do I receive an ID card? How do I order replacement or extra cards?

It’s important to know that you do not need an ID card to access your vision benefits. Simply select a participating provider, call for an appointment and tell them that NVA administers your vision program.
However, if your plan provides ID cards, you will automatically receive two ID cards with the subscriber’s name listed. If you need replacement or extra cards, you may log in and print them from this website or log in to the app and flash your ID card at time of service. If your plan does not use this option, you can call our 24/7/365 customer services center at 800-672-7723 or e-mail us at service@e-nva.com to order ID cards.
- Can I apply Flexible Savings Account (FSA) or Health Savings Account (HSA) funds to out-of-pocket costs after my NVA benefit is applied?

You sure can! You can use your FSA or HSA dollars to pay your out-of-pocket expenses, including eye exam copay, out-of-pocket costs for prescription eyeglasses or contact lenses. You can even use them for LASIK surgery. Be sure you understand the rules of your particular account.
NVA Smart Buyer® Tip
Ask your eye doctor to measure your pupillary distance (PD), the distance between the center of each of your pupils. You’ll need this information if you plan on purchasing your eyeglasses online. Some eyeglass online retailers provide you instructions on how to measure yourself. However, your eye doctor can measure more accurately. Some eye doctors may charge for this service and some online retailers may reimburse you up to a certain amount for the PD measurement charge when you order your eyeglasses from them.
