The following pages list vision care providers and facilities that participate in our network programs. We make every effort to ensure that these listings are accurate. However, additions, deletions, and changes will occur over time. You can verify a provider's status by calling the provider directly. If you experience a problem contacting a provider, please call 800-672-7723.

Important Notice: Any physician included in this directory is listed for outpatient office visits. In addition, the directory includes information regarding whether the provider is currently accepting new patients.

PROVIDER NETWORK SELECTION CRITERIA

The NVA national network of eye care providers includes independent providers, retailers and optical shops that provide routine eye examinations and optical materials. Our members have access to these providers at any time. NVA builds its network based on the availability of providers in any given area and the ability of those providers to meet NVA’s credentialing participation standards. Providers that meet NVA’s participation standards receive a contracted reimbursement rate for each service they provide. Providers may agree to accept fixed pricing on non-covered services and materials, which limits members’ out of pocket expenses. NVA works to recruit providers recommended by members and clients to ensure quality providers are available.

NVA surveys its members on an annual basis to assess satisfaction with NVA’s vision care network. Further, between credentialing cycles, as part of our ongoing quality review process, NVA enrolls its providers in the National Practitioner Data Bank (NPDB) ongoing monitoring program, which alerts NVA of any applied sanctions, malpractice cases, or changes in licensure status.
