Welcome Providers

Registration is easy! After you create an account, you'll have access to:

View subscriber eligibility for services and eyewear
Plan design information
Request authorizations for those members who are eligible
Submit claims electronically
View your claims payment history

You must have received your NVA welcome packet e-mail that includes your suffix code and be a participating provider to register. If you are not an NVA provider and would like to become one, please select Join Our Network and submit the required information to apply.

Tax ID#: Please include the same Tax ID number you included in the New Provider Enrollment form. If you don't remember the Tax ID number you were provided, call 1-800-672-7723 to retrieve it.

Suffix Code: The suffix code is found in the NVA welcome packet e-mail. If you can't find your NVA suffix code, call 1-800-672-7723 to retrieve it.

Zip Code: Please include the zip code you included in the New Provider Enrollment form. If you don't remember the zip code you were provided, call 1-800-672-7723 to retrieve it.
